FIRST M.B.B.S. ADMISSION FOR THE ACADEMIC YEAR 2008-09
General Instructions for newly admitted I M.B.B.S students (July 2008 Batch)
IMPORTANT INFORMATION AT GLANCE
Congratulations on your admission to the GS Medical college and a hearty Welcome.!!
Please follow the instructions below carefully, enable your admission formalities to be completed easily.
1. Submit one photocopy of the selection letter at counter No.02.
2. Register your details in the register at counter No. 02 & 04.
3. Collect photocopy of the selection letter from counter No. 02 duly stamped for collection of Admission form which is available on cash counter of college office on payment of Rs.520.00 between 11.00 a.m. to 3.30 p.m. on Monday to Saturday except second and fourth Saturdays and holidays.
4. Fill up the B.C. free ship form at counter No.08 (For reserve category students)
5. Arrange following original and attested Xerox copies of five set of each certificates/documents in following sequences in the thick file.
01) Admission Form (Green Form)
02) A) Nationality certificate OR
B) Valid Passport OR
C) Domicile Certificate
(issued by District Magistrate / Metropolitan Magistrate / Addl. District Magistrate or Tahsildar. (With Original Birth Certificate & Attested xerox copy of Ration Card)03) 10 th Mark sheet & Passing certificate
04) 12 th H.S.C. Mark sheet
05) MH-CET Mark sheet (for MH-CET Students)
06) AIEE Rank letter & Selection Letter (For All India Students)
07) Physical fitness certificate
08) Caste Certificate
09) Caste Validity certificate
10) Non Creamy Layer Certificate (For NT-1, NT-2, NT-3, VJ, OBC,)
11) Migration Certificate
12) College Leaving OR Transfer Certificate
13) Self Educational Gap certificate (Affidavit from Student)
14) MH-CET / AIEE admit card & Receipt cum Identity Card.
15) Original Birth certificate
16) Xerox Copy of Ration Card.
17) Xerox copy of Parents income Certificate
18) Undertaking
6. Submit this file and get the same verified from counter No.02 and Collect the Payment slip for payment.
7. Fees will be accepted at cash section of the college office subject to provisional eligibility from the Registrar, Maharashtra University Of Health Sciences Nashik and without prejudice to final court order in case admission is related to writ petition where in interim court order are issued and subject to submission of agreement bond with in 15 days from the date of issue .
DETAILS OF THE FEES.
i) A) One D.D./Pay Order of Rs.21,600/- towards tuition fees, Library fees etc. and another D.D./Pay Order of Rs.2,000/- towards deposit (for AIPMTand MHT-CET STUDENTS from COMMON CATEGORY)drawn from any Nationalized Bank in favour of Municipal Corporation of Greater Mumbai.
B) One D.D./Pay Order ofRs.2,400/- towards fees, library fees etc. and another D.D./Pay Order of Rs.2,000/- towards deposit (for candidate is from RESERVED CATEGORY & O.B.C. CATEGORY) drawn from any Nationalized Bank in favour Municipal Corporation of Greater Mumbai.
ii) Rs.600/- towards enrollment and eligibility fee. (if the candidate is from MHT-CET) and Rs.1,050/- towards enrollment and eligibility fee (if the candidate from AIPMT) by demand draft/pay order drawn on any Nationalized Bank in favour of Dean, Seth G. S. Medical College .
iii) Rs.540/- towards Gymkhana fees and Welfare Fund (for AIPMTand for MHT-CET STUDENTS)by Demand Draft/ Pay order drawn on any Nationalized Bank in favour of Seth G. S. Medical College, Gymkhana.
iv) Rs.104/- towards Service Agreement Bond Form charges, by cash (for AIPMTand for MHT-CET STUDENTS)
8. Submit 2 photocopies of payment receipt, at counter No.04.
9. Collect the service Agreement Bond (Major or Minor as the case may be from counter No.04 on submission of photocopy of the payment receipt of Rs.104.00.
10. For railway concession please contact at counter No.9, students section with Identity card.
11. Submit NTS & NMS forms after admission along with sanction letter at counter No.4 of students section.
TEACHINGPROGRAMME
For further details on teaching programme/registration no. candidates are directed to report to the Anatomy department, 2 ndfloor of this college. And for SHIDORI contact to Mr.Rajan, Pharmacology department, 1 st floor.
HOSTEL FACILITIES FOR BOYS AND GIRLS
Due to change in admission rules number of students from outside Mumbai has been increased, therefore demand for Hostel accommodation has also increased. In these circumstances it is not possible to consider all the requests for Hostel accommodation. The request will be considered on the basis of availability of the accommodation.
BOND
Students are requested to go through the instruction carefully before filling the SURETY BOND AND THE SERVICE AGREEMENT. The students seeking the admission for the year 2008-09, have to execute the Service Agreement / Surety Bond of Rs.10,00,000/- (Rs.Ten Lakh only).
Submit the Following documents along with agreement bond in respect of sureties.
1) Income Tax Returns with Income certificate stating that income is, more then One Lakh.
2) Employment certificate, Employment Identity card,(Certified Photo Copy)
3) Copy of Pan Card.
4) Photocopy of the ration card/ Telephone Bill/Rent receipt/Assessment receipt/Shop & Establishment certificate/ License etc.
5) Recent passport size Photograph.
6) In case of self employed person not having either income tax return pan card
IDENTITY CARD
Identity card will be issued only after completion of all admission formalities including submission of Service Agreement Bond & Surety Bond with all requisite documents on Counter No.4.
PLEASE NOTE: Students are requested to bring their own stationery (i.e. stapler. gum bottle, blank papers, paper pins, punch, pen etc. with them. We are sorry; but, college will not be able provide any stationery to the candidates.
For admission, the student and one parent should be present.
The bond amount is Rs. 10.0 lakhs for ALL students.
