Background History


The Department of Obstetrics and Gynaecology offers comprehensive services for the reproductive health and gynaecological needs of women. It has one of the highest workloads in the entire country and admission into the residency training programme is highly coveted. Being a tertiary care centre, patients are referred from all over with a fascinating array of obstetric and gynaecological conditions.
The annual footfall of the outpatient department including new and old patient visits is around 50,000 with 12,000 inpatients and 4000 surgeries. Annually, around 7,000 women deliver in this centre. Large proportions of these women have some high risk factor complicating pregnancy. Critical
care and medical disorders in obstetrics are areas of special focus. Babies requiring intensive care are transferred to an excellent Neonatal Intensive Care Unit. Complete contraceptive services and facilities for medical termination of pregnancy are also available. Microsurgery and endoscopic surgeries are
performed too. There is a vigorous teaching programme for undergraduate and postgraduate medical students as well as nursing trainees. The department also accepts observers through the programme offered by the college. Research is actively encouraged and the department has actively participated in various pioneering academic studies. The department also conducts periodic CMEs in accordance with the Maharashtra Medical Council guidelines. The department has intake of 36 MS Postgraduate students. Twenty one are posted in KEM Hospital and fifteen are posted in the Nowrosjee Wadia Maternity Hospital (both of which are affiliated to the Seth GS Medical College). New batch of postgraduate students are given the choice of selecting their post graduate guides as per merit.

Information for Patients
The Department runs daily outpatient clinics for gynaecological patients in the morning and obstetric patients in the afternoon. New
registration papers are issued between 8.00am and 10.30 a.m. and 12.30 noon and 2.00 p.m. Emergency care is available all day and night. The Gynecology Outpatient Clinic is held every day from 8.30 a.m. and 12.30 p.m. in Clinic No. 109. The case papers are available only between 8.30 a.m. and 10.30 a.m.

The following services are offered at this clinic.

  • Pap smear for screening of the genital tract cancer.
  • Endometrial aspiration cytology for screening for uterine cancer
  • Gynecologic examination.
  • Counseling.
  • Colposcopy.

The Infertility Outpatient Clinic is held every day from 10.30 a.m. and 12.30 p.m. in Clinic No. 109A. Patients referred to this clinic from the Gynecology clinic can attend it on any day other than Sunday. The following services are offered at this clinic.

  • Ovulationstudy.
  • Cervical mucus study.
  • Postcoital test.

The Obstetric Outpatient Clinic is held every day from 1.30 a.m. and 4.00 p.m. in Clinic No. 109. The case papers are available only between 12.30 p.m. and 3.00 p.m. The following services are offered at this clinic.

  • Antenatal care.
  • Pap smear for screening of the genital tract cancer during pregnancy.
  • Blood testing for hemoglobin, blood group, VDRL, and HIV after counseling.
  • Colposcopy.

The Family Planning Outpatient Clinic is held every day from 9.00 a.m. and 12.30 p.m. in Clinic No. 109. The case papers are available only between 7.30 a.m. and 10.30 a.m. The following services are offered at this clinic.

  • Counseling for contraception.
  • Family planning methods like sterilization, intrauterine device insertion, condoms, combination contraceptive pills, Depot medroxyprogesterone injections etc.
  • Follow-up examination and care for users of contraceptive methods.
  • Medical termination of pregnancy.

The Postnatal Outpatient Clinic is held every day from 1.30 a.m. and 4.00 p.m. in the Family Planning Center. The case papers are available only between 12.30 p.m. and 4.00 p.m. The following services are offered at this clinic.

  • Postnatal care to mothers.
  • Counseling for contraception.
  • Colposcopy

Residency Training Programme

Result
The residency training programme is one of the most sought after and admissions are made on the basis of existing regulations laid down by the state government of Maharashtra and the Mumbai municipal corporation. The postgraduate students for M.S. programme are posted in King Edward Memorial Hospital and Nowrosjee Wadia Maternity Hospital on merit. The residency begins with an exhaustive orientation in the first year covering aspects of residency, medical, ethical and medicolegal aspects of training and patient care. The postgraduate training program is exhaustive. It includes didactic lectures which cover the entire syllabus in each unit over three years, small group teaching, maternal mortality audits, perinatal mortality audits, journal clubs, seminars, symposia, case discussions, and quiz programs. Training on operative techniques is both extensive and intensive.  Special training of the first year residents with refresher sessions for other doctors includes comprehensive care of all medicolegal cases including survivors of sexual assault. Sessions by lactation consultants on ‘training in lactation’are regularly conducted.

This department has a very comprehensive residency training program. It is both mentally challenging and physically taxing. Resident doctors are expected to awake at the crack of dawn and see patients before the treating physicians arrive. The residency years can be gruelling, as it requires that residents take call and spend the entire day and night in the hospital to care for sick patients. Each year the resident is allowed increasing responsibility and to attempt more difficult procedures and surgeries. The training is both extensive and intensive and also, becoming a skilled surgeon takes practice. Patients coming to KEM Hospital are usually the sickest patients in and around Mumbai. Residency is both very difficult and very rewarding. The resident has to spend countless hours caring for sometimes hostile and violent patients, dying patients, counselling parents if their baby has no heart beat, as well getting the joy of bringing babies into this world and saving lives of many babies and their mothers. A young doctor who chooses a career in obstetrics and gynaecology should have a passion for the specialty, since the hours during training are painfully long. During the residency years, the M.S. resident has also to submit a Dissertation which is made after detailed research and review into the topic undertaken under his/her Guide. This is a demanding process.
The academic session for residents also includes preparing and presenting seminars, teaching the medical school students, taking part in various workshops and conferences and also publishing one’s research work. The resident is held accountable for the way he/she manages a patient in the regularly held maternal and perinatal mortality and morbidity audits in the department.’

Resident Involvement Program
Post Graduate Seminar Database: Seminar topics are selected, researched by a team of 6-8 residents and then it is presented by them to the department post graduate residents, students and faculty members. In order to instill in them administrative skills resident doctors are part of

In addition to the Gynecologic and Obstetric Research Center, the department also has access to other laboratories in the hospital, making advanced research possible. Clinical research is done on different aspects of Obstetrics and Gynecology all-round the year. Clinical research is also done in association with other departments of the hospital, and institutes like Institute for Research in Reproduction and ICMR. A large number of drug trials are done in association with Pharmaceutical companies. All research activity is carried out only after approval from the Ethics Committee of the institute.

These are evidence-based, based on the standard postgraduate books of obstetrics and therapeutics, and literature survey. They are meant to help manage the patients effectively. The use of these recommendations is optional. Modes of management other than those described may be used to suit different patients. The decision of the qualified doctor treating each patient will be final.

The department offers services for family planning counseling and methods. The outpatient clinic is held every day between 9.00 a.m. and 12.30 p.m. The services offered are contraceptive counseling, medical termination of pregnancy, and contraceptive methods like sterilization, intrauterine device insertion, condoms, combination contraceptive pills, and depot medroxyprogesterone injections.
