Department gained formal shape as we know now by year 1957.This department is one of the largest teaching departments and is a busy department which handles outdoor and indoor Internal Medicine patients and also patients attending the Emergency Ward (Emergency Room). Total numbers of IPD beds are 350 with around 29574 admissions last year and average OPD patient load of 70000 patients.
In addition, training is imparted to 250 MBBS students and 23 post graduate MD students every year.
Milestones:-
Apart from general medicine OPD 26, department of Medicine has various speciality OPDs including Rheumatology services since 1977, Hypertension services since 1990, Geriatric services since 1992, Virology OPD since 1999 and IDSP OPD for infectious diseases since 2006.
Emergency medical services were established and expanded under the tenure of respected Dean Dr. Shirhatti in year 2000 and further expanded in year 2022 to 15 ICU beds and 12 Emergency beds. MICU was upgraded in September 2022 to 26 bedded fully equipped ICU.
Achievements:-
Faculty and post graduate students have received various national and university awards in national conferences held all over the India.
Events:-
Faculties from Department of Medicine have organize various local and state level CME and training courses for post graduate students and physicians. Medicine department was first department to start the foundation course which is effectively running since last 8 years.

The outpatient clinic is conducted daily from Monday to Saturday each unit consults one day per week.
Patients attending the OPD for the first time will have to get an OPD case paper which is issued at the registration counter outside the OPD (026) from 7.30am to 10.30am. Only patients who have been specifically referred to the Medicine OPD from other OPDs or by the family doctor or private consultant will be issued OPD paper. Patients who have not been referred to Medicine OPD are first seen in the General Practice OPD (OPD No 006) and if necessary, referred to OPD 026.
Patients attending OPD for second and subsequent visits will have to get their case papers stamped at the registration counter and then attend OPD. Patients with case papers of a particular unit will only be seen on the OPD day of that particular unit.
Patients requiring elective (no urgent) hospital admission are admitted from the OPD.
Average work load:
In the year 2022 a total of 45506 patients (2965 new patients and 42541 follow-up cases) attended the OPD.

Emergency Services:-
The Emergency Ward (Ward No 20 – emergency room) has 15 ICU beds and is located on the ground floor next to casualty. This is staffed by lecturers and resident doctors round the clock. Patients requiring urgent attention are first seen by the casualty officer and then referred to the emergency ward. Here, they are evaluated by the doctor on duty and a decision is made regarding admission to the various wards like general ward, Medical ICU, ICCU etc. Specialist opinion may also be obtained prior to admission. Emergency cases – the very seriously ill, are initially admitted to this ward and stabilized before being shifted to other areas of the hospital.

In patient Services:-
Patients requiring hospital admission are admitted either from the Out Patient Clinic, or from the emergency room. Patients admitted from the OPD are admitted under the OPD unit. All patients attending the emergency services on a particular day are admitted under the unit that is on emergency duty. If they have old case papers of another unit (OPD or indoor) then they are subsequently transferred under the care of their original consultant.

Average work load:
Approximately 60 patients are admitted to the medical wards every day on emergency basis and 8 from the outpatient clinic. In the year 2021 – 2022, 29574 patients were admitted, with 3205 deaths.

Established in the year 1977. In the year 2022, 4196 new patients and 15330 old patients attended the OPD and 355 patients required indoor admission
Rheumatology OPDs and review clinic are conducted twice a week. In addition Scleroderma, backache and lupus clinics are held every week. Patients are offered medical care, education about their disease and Rehabilitation therapy in collaboration with the Occupational Therapy, Physiotherapy and Social Service Department
An Immunology laboratory (Department of Pathology) with facilities for conducting tests like RF, CRP,ANA, dsDNA, A.N.C.A., A.C.L.A.,HLA B 27, estimation of C3, C4, and Synovial fluid analysis is available. Registries for Rheumatoid Arthritis and Seronegative Spondyloarthropathy have been maintained.

OPD:
The clinic has an attendance of 5657 patients (including 866 new cases and old cases 4791 seen in 2022) every year. Patients are investigated for essential and secondary hypertension, and individualized therapy is provided. Patients are closely monitored for complications. Consultancy services are provided for hypertensive patients referred from various specialty wards and clinics of the hospital.

Workload:
Established in 1985 as a 7 bed ICU, in 1993 this was expanded into a 17 bed ICU and renamed as Jayantilal Chandulal Mehta Medical – Neurology – Neurosurgery Intensive Care Unit. This is a multidisciplinary ICU which is located on the second floor of the main building. Looking at the need for critical patients MICU is Further expanded to 26 beds on 29th Sept 2022.
Patients from medical, Obstetrics, neurology, neurosurgery and neuroradiology are referred to the unit for intensive care. The patients admitted to the unit are attended to round the clock by 1 senior resident and 1 junior resident from the Medicine department. In addition, residents from neurology and neurosurgery are readily available for consultation. The unit gets about 1000 to 1100 admissions annually and provides advanced intensive care to patients with a wide variety of medical and tropical diseases including severe malaria, tetanus, leptospirosis, poisoning, snakebite, scorpion sting, Guillain Barre syndrome, myasthenia gravis, subarachnoid hemorrhage, Status epilepticus and cerebral cortical venous thrombosis, Ecclampsia and other obstetrics emergencies.
The ICU is in the process of acquiring equipment for full invasive hemodynamic monitoring, Bronchoscopy and a dialysis machine for continuous renal replacement therapy.
The MICU regularly gets observers from different countries who require exposure to acute medicine in a tropical setting.
Average work load:
Approximately 400-600 patients are referred here every day and 80-100 patients require admission to the emergency ward. In the year 2022-2023 more than one lac patients were referred to the Emergency room for treatment and more than 2000 patients were admitted in the emergency ward
Patient feedback and education:
Informal feedbacks are taken from the patient and relatives regarding the treatment. Various camps are conducted by Department of Medicine for education of community about chronic illnesses. Patient education booklet and pictorial exhibitions are prepared.

Dr. Santosh Salagre and Dr. Smrati Tiwari are faculty for GSMC FAIMER for fellowship and Dr. Santosh Salagre is also faculty for Philadelphia FAIMER fellowship.
MUHS approved Fellowship in Critical Care in one year and Fellowship in Infectious Disease for one year.
